This is an excellent opportunity for nurses who want to transition from traditional clinical roles to a care coordination and case management environment — no prior managed care experience required! You will work with patients, families, and providers to coordinate care, support recovery, and ensure the highest quality outcomes. Training and mentorship will be provided to help you succeed in this rewarding field role. ✅ Key Responsibilities • Conduct in-home or field visits to assess patient health status and care needs • Coordinate care plans and ensure continuity of care among providers • Educate patients and families on treatment plans and community resources • Collaborate with healthcare teams to ensure effective case management • Maintain accurate and timely documentation • Travel locally within assigned NJ counties (fuel reimbursement provided) Qualifications • Active Registered Nurse (RN) license in New Jersey • Strong clinical assessment and communication skills • Compassionate, organized, and patient-focused • No managed care experience required — training provided • Valid driver’s license and willingness to travel within the region Interested?
